Before using the from below, I will explain a little how it works:
Once you fill in the from and hit "Add" you will see a confirmation showing you that your comment has been added, and if you click back, you will see your comment at the top of the list.
The "Name", "Comments" and "Enter Code" fields are all mandatory. (No need to worry about caps with the "Enter Code" field.
It is optional to use the "e-mail" & Website URL" fields, as they will also be shown with your comments when listed.
**Please note that all website URL's must include "http://" at the start, and that no links or URL's may be include in the "Comments" field.**
